Guildnet MLTC, FIDA and Medicaid Advantage Plus plans are closing as of January 1, 2019.
Official notices are being sent to all Guildnet members in mid-October 2018.
What You Need to Know
♦ You will receive a notice from New York Medicaid Choice advising you to choose a new plan.
♦ You will have 60 days to select and enroll in a new plan.
♦ If you do not enroll in a new plan, you will be auto-assigned to a plan.
Know Your Rights
Consumer “Transition Rights” entitles you to the same services (same number of hours and days per week) for 120 days from the new plan that you were receiving from Guildnet. However, you only receive those rights by making a switch after you receive an official notice from Guildnet, NY Medicaid Choice or the NYS Department of Health.
